What does it take for a coin to be designated xxxx D/D or S/S or D/S for repunched mintmarks? Is there a level of magnification that it must be seen at/ or degree of separation, and must it be a commonly recognized coin and series before any designation will be given?

In general the over mintmark most be easily visible with 5X magnification is the recognized variety. For some issues, there are several "states" of the repunch. In general, the strongest state, i.e. the most obvious, boldest, etc., is the one recognized by the numismatic community. The number of sates and the acceptable states vary from issue to issue. Sometimes an issue will have several states that are recognized. It's really a matter of each issue having specific recognition guidelines, rather than a general guideline that applies to all over mintmarks.
Good question, though my answer is not very precise...nature of the beast.
